This is the individuals registration form for our “Summer Sign Off” Coed Grass 4's Volleyball Tournament, taking place at the Haysboro Community Association on Saturday, September 5th from 10:00am to approximately 5:00pm.

Please note - as you are signing up as an individual, submission of this form does not guarantee placement on a team. We will be in touch shortly after submission to confirm your status on a team.
